Please fill out the application below to determine the athlete’s eligibility to receive a scholarship from the Love 11 foundation.
Once you are finished with the application, please upload a digital photo of the athlete applicant here (insert the link to the upload page).
We use the photos to help raise funds for future athletes in need of assistance.
Once receiving your application, our board will then vote/assess if the scholarship is approved.
After voting, the family will be contacted regarding the approval outcome.
If approved, we will mail out the awarded funds directly to the sports business requested on the application.
The parent will be responsible for the $25 commitment fee outlined on the application.
This fee will be paid by the parent directly to the sports organization and never directly to Love 11.
